People weighing Icacos are usually also looking at Vieques and Culebra, the two inhabited islands off Puerto Rico's east coast, and at Palominos, the cay next door. They are not the same kind of trip.
Icacos is a half day or a day from Fajardo with no facilities at all. Vieques and Culebra need a ferry from Ceiba or a small plane, and they reward a night or two. Palominos belongs to a resort. Here is how to choose.
The quick answer
With one free day and no wish to change hotels, go to Icacos. With two or more days and a taste for small towns, go to Vieques for the bioluminescent bay or Culebra for Flamenco Beach. Palominos is private; you see it on a charter that chooses it, or as a guest of El Conquistador.

Icacos or Vieques: which should you pick?
Icacos for an easy day from Fajardo; Vieques if you can stay at least a night. The table sets out the differences.
Ride times and facts come from Discover Puerto Rico and Wikipedia unless marked.

One day, no hotel change: a beach stop, a reef swim and lunch on a catamaran from Puerto del Rey.
Book thisWhy is Vieques so popular?
Mosquito Bay, wild horses and a big wildlife refuge. Discover Puerto Rico calls Mosquito Bay the brightest bioluminescent bay in the world.
Discover Puerto Rico sums up Vieques as a pristine wildlife refuge with wild horses, a black sand beach called Playa Negra, and Mosquito Bay, where microorganisms glow blue when the water moves. Small towns, dark skies and fewer crowds do the rest.
That glow is the reason many people stay overnight: the bay tours run after dark, which makes a day trip awkward. If the bio bay is what you are after but you cannot spare a night, Fajardo has its own: the Laguna Grande kayak tour leaves from Las Croabas at sunset, the same lot as the Icacos water taxis.
Cayo Icacos or Flamenco Beach?
Flamenco for a beach with services and a town; Icacos for a short ride and nothing built at all.
Flamenco Beach on Culebra curves for about a mile around a horseshoe bay sheltered by a reef. Wikipedia lists bathrooms, showers, food kiosks, lifeguards and a camping area, plus two rusting tanks left by the US Navy. It is part of the Culebra National Wildlife Refuge.
Icacos has none of those services, and no lifeguards. What it has is a 15-minute ride from Fajardo and a day that ends back at your hotel. Flamenco needs a ferry from Ceiba of 45 minutes or more, a transfer across Culebra, and a seat on the return boat. As a day trip from San Juan, Flamenco is a long day. Icacos is not.

How do you get to Vieques and Culebra from Ceiba?
By ferry from the Ceiba terminal, run by the Maritime Transport Authority, or by small plane from San Juan.
Per Discover Puerto Rico: separate ferries run from Ceiba to each island, about 30 minutes to Vieques and about 45 minutes to Culebra (its FAQ gives longer times, so allow for them). Buy tickets on the authority's site, book ahead for weekends and holidays, and arrive at least an hour early. There is no ferry from San Juan, and the Ceiba terminal is about 1.5 hours' drive from the city. Flights from San Juan take about 25 minutes to Vieques and 30 to Culebra.
No public ferry runs between Vieques and Culebra. The new Ceiba passenger terminal is covered in our Ceiba ferry terminal post.
Icacos or Palomino?
Icacos, unless you are staying at El Conquistador or booking a private charter. Palominos is a private island used by the resort.
Palominos sits east of Icacos in the same chain. Wikipedia lists it among the privately owned cays, outside the protected ones. Resort guests reach it in under 10 minutes by boat.
For everyone else there are two ways to see it. The private half-day charter lets you pick Icacos or Palominos on the day. The full-day catamaran may switch to Palominos or Lobos when the captain decides; you do not choose.

Up to six people, your own boat, and Icacos or Palominos chosen on the day. Snorkel gear, drinks and snacks on board.
Book this Tour detailsCan you see more than one island in a day?
Not easily. Icacos tours stop at one cay, sometimes after a reef, and no public ferry links Vieques and Culebra.
A tour boat to Icacos might visit a reef and a cay, and the full-day catamaran may pick a different cay, but you will not tour the chain. The snorkel cruise with slide is described as an Icacos trip; its web address also mentions Vieques, so glance at the itinerary on the booking page.
To combine islands, base yourself in Fajardo: Icacos one day, a ferry from Ceiba to Vieques or Culebra another.

Is 3 days enough for Puerto Rico?
Enough for San Juan, one island day and one night tour. Not enough to add Vieques or Culebra without rushing.
Three days works like this: a day in Old San Juan, a day on Icacos with the bio bay kayak tour in Laguna Grande that evening, and a day for El Yunque or a rest. Fajardo is about an hour from San Juan, so the island day fits without changing hotels.
Vieques or Culebra on a three-day trip means a ferry from Ceiba and a night there, which eats a day on each side. Save them for a longer stay.

Finish the island day in the dark: paddle a mangrove channel at sunset into Laguna Grande, from the same Las Croabas lot.
Book this Tour detailsWhen is the worst time to go?
Hurricane season, 1 June to 30 November, peaks around 10 September, according to NOAA. Weekends on Icacos are the other thing to avoid.
NOAA puts the Atlantic peak around 10 September, with a secondary mid-October peak for the Caribbean. Trips run through the season, but plan a spare day; the 2026 hurricane season post covers what happens to bookings.
Discover Puerto Rico rates December to April as the best stretch, and weekdays as quieter than weekends. On Icacos, skip the weekend if you can: the party boats come out.

A half day with a reef and a beach, morning or afternoon, so you can dodge the weather.
Book thisFrequently asked questions
Is Icacos better than Vieques?
For a day trip from San Juan or Fajardo, yes: the ride is about 15 minutes and you are back by evening. For the bioluminescent bay, wild horses and a night on an island, Vieques wins.
Can you visit Palomino Island?
Palominos is private and used by El Conquistador resort guests. A private charter from Sunbay Marina can choose Palominos, and the full-day catamaran sometimes stops there.
Is Culebra or Icacos easier from San Juan?
Icacos. It is about an hour to Fajardo and a 15 to 20 minute boat ride. Culebra needs a drive of about 1.5 hours to Ceiba and a ferry of 45 minutes or more, or a flight.
Can you do Vieques as a day trip?
Yes, by ferry from Ceiba or by plane from San Juan, but it is a long day, and the bio bay tours run after dark. A night there makes more sense.
